Prevent Drill Dentistry in Brisbane: Stops Tooth Decay Without Drill-and-Fill

Snakes. Spiders. Sharks. These are things that strike fear and dread in most people. And one more … the sound of a dentist’s drill!

Imagine a world with no dentist drills. Sounds like the opening line to a science fiction film, right?

It doesn’t have to be fantasy, though.

Modern dentists have been thinking about how to take the “drill and fill” out of dental care for decades.

It’s possible that your children may live their entire lives without experiencing the dreaded drill.

Tooth Decay

Dentists and scientist have been struggling for a long time to understand exactly what causes dental caries (caries is the trade name for tooth decay and cavities) and how to stop it.

Today we have a pretty clear understanding of the chemical processes that cause tooth decay.

It is the same basic chemical process that happens in all organic decomposition everywhere, except that it is happening in our mouths, and the acid by-products work to destroy our precious teeth.

By the time that we see tooth decay happening, the process is already quite advanced.

The question of how to detect and stop the decaying process before it requires invasive, traumatic drilling and filling is what modern dentistry has been trying to answer.

No-Drill Dentistry and the Caries Management System

Minimum Intervention Dentistry (MID), Atraumatic Restorative Treatment (ART), and Caries Management System (CMS) are names for schools of thought and systems with the common goal of providing dental care that ends the need for the dreaded drill and fill dentistry that is so odious to most people.

The basic idea behind these schools of thought is that dental decay is not inevitable and that it is a disease that can be cured through:

  • Close attention to the patient’s individual dental conditions
  • Early detection and treatment of nascent caries
  • Intensive training in preventative homecare

The Caries Management System (CMS) is a ten step, non-invasive strategy to arrest and remineralise early lesions.

This system was developed for use by general practitioners following a new Caries Management Policy that had been adopted by the Faculty of Dentistry, University of Sydney.

The ten steps eliminate the need for traumatic drilling procedures, unless the patient is already suffering from acute tooth decay that leaves no other option.

The 10 steps of CMS are as follows:

  1. Diet assessment
  2. Plaque assessment
  3. Bitewing radiographic survey
  4. Diagnosis and caries risk assessment
  5. Preparation of treatment plan
  6. Case presentation at which patient is informed about:

    • Dental caries:

      • Arrest
      • Reversal/natural repair (remineralisation)
      • Prevention
      • Number and status of current lesions
      • Role of dental practitioner in caries management
      • Role of home care in caries prevention
      • Current caries risk status
      • Result of diet assessment and recommendations
  7. Oral hygiene coaching
  8. Topical fluoride application (both professional and home care)
  9. Monitoring of plaque control and treatment outcomes at each visit
  10. Recall program tailored to caries risk status

For everyone else, getting into the CMS routine of preventative homecare and lifestyle, combined with early diagnosis and treatment of caries could mean that the drill might be a nightmare of the past.
