No-Drill Dentistry and the Caries Management System
Minimum Intervention Dentistry (MID), Atraumatic Restorative Treatment (ART), and Caries Management System (CMS) are names for schools of thought and systems with the common goal of providing dental care that ends the need for the dreaded drill and fill dentistry that is so odious to most people.
The basic idea behind these schools of thought is that dental decay is not inevitable and that it is a disease that can be cured through:
- Close attention to the patient’s individual dental conditions
- Early detection and treatment of nascent caries
- Intensive training in preventative homecare
The Caries Management System (CMS) is a ten step, non-invasive strategy to arrest and remineralise early lesions.
This system was developed for use by general practitioners following a new Caries Management Policy that had been adopted by the Faculty of Dentistry, University of Sydney.
The ten steps eliminate the need for traumatic drilling procedures, unless the patient is already suffering from acute tooth decay that leaves no other option.
The 10 steps of CMS are as follows:
- Diet assessment
- Plaque assessment
- Bitewing radiographic survey
- Diagnosis and caries risk assessment
- Preparation of treatment plan
- Case presentation at which patient is informed about:
- Dental caries:
- Reversal/natural repair (remineralisation)
- Number and status of current lesions
- Role of dental practitioner in caries management
- Role of home care in caries prevention
- Current caries risk status
- Result of diet assessment and recommendations
- Dental caries:
- Oral hygiene coaching
- Topical fluoride application (both professional and home care)
- Monitoring of plaque control and treatment outcomes at each visit
- Recall program tailored to caries risk status
